The 2026 GCSG conference in San Diego felt different than previous years.

The tone was grounded in a heavy sense of pragmatism because our industry is operating under an extreme level of structural strain. Across every session, we heard supply leaders from the largest pharmaceutical organizations describe the same recurring challenges regarding forecasting, labeling, and distribution.

While the presentations were rich with data about these pain points, the focus remained on managing operational symptoms. As an industry, we are currently spending more energy trying to guess the future than we are building a system that can respond to the present.

It is time to look beyond the symptoms and treat the root cause. We must move away from a speculative release model that puts 40 percent of your supply value at risk before a patient is even confirmed. We need a shift to a supply architecture that is driven by events and professional pharmacy practice.

In October 2025 we held the first ever un-conference for pharmacists. It was an insightful event and we were delighted to see the interest and the participation and willingness of pharmacists to engage, learn and teach.

Clinical trials are the backbone of modern medicine, serving as the essential gateway through which new treatments must pass to ensure they are safe and effective. These trials are critical for researchers applying to the FDA for commercial approval.

Yet, the universal challenge in clinical research today is finding participants who qualify and will commit to the entire trial period, which leads to high dropout rates.

We sat down with clinical trial expert and licensed pharmacist Austin Tull to peel back the curtain. Tull, who previously oversaw a sizable clinical trial site with about 100 active studies, explains that high dropout rates are often a result of patients being brought into impersonal clinical trial sites without any previous relationship.

The innovative solution lies in expanding the involvement of community pharmacies.

The future of clinical research hinges on community engagement, and pharmacists hold a powerful, unique position to drive this change. Their accessibility, trusted relationships, and frequent patient interactions are solving major barriers in clinical research.

📍 The High-Value "Trust Unit": While doctors and nurses are highly trusted, pharmacists combine moderate-to-high trust levels with significantly higher visit frequency (patients visit 4 to 6 times yearly). This combination creates a large "trust unit" or "Trust & Loyalty Scale" score (around 8 out of 10) that multiplies their influence over patient decisions.

📍 Solving Recruitment and Retention: Because patients already trust their pharmacists and interact with them frequently, their involvement in community-based trials increases willingness to participate and improves patient adherence and retention. Pharmacists are able to address concerns, explain protocols, and provide ongoing support.

📍 Driving Diversity and Decentralization: Community pharmacies, embedded in neighborhoods, are perfectly placed to reach populations often excluded from traditional trials, including rural, elderly, and minority groups. This decentralized model, using community pharmacies as main hubs and satellites, accelerates enrollment and makes trials more patient-friendly.

This shift leverages the pharmacist's clinical expertise and trusted patient relationships to improve trial quality, expand patient diversity, and accelerate medical innovation.

🙌 Unleashing the UN-stoppable Power of Pharmacy: Major Takeaways from the RxE2 Un-Conference!

Forget standard industry seminars. We just wrapped the inaugural RxE2 un-conference—the first of its kind for pharmacists in clinical research—and the collaborative energy and insights were truly UN-precedented!

Held during American Pharmacists Month, this event adopted an uncommon format: it was participant-driven, meaning attendees created the agenda and fostered open discussion and sharing of ideas.

The central theme was clear: Community pharmacists can radically impact clinical research, enhance patient access, and, critically, unlock additional, unconventional revenue streams for their pharmacies.

📍 Key Insights into UN-questionable Trust:

*️⃣ Ethical Pillars: Dr. Donald Miller (North Dakota State University School of Pharmacy Professor Emeritus) laid the ethical foundation, stressing that a clinical trial is the most ethical way of finding out the benefits and risks of any intervention. He even noted that using a placebo is entirely ethical if there is “true doubt” about a new drug's safety and efficacy.

*️⃣ Unrivaled Credibility: Marc Ost (Eric’s RX Shoppe co-owner/CEO) emphasized the immense value of the community pharmacy's unquestionable credibility and the "direct relationship" they hold with patients for referrals.

*️⃣ The Unmatched Trust Unit: Gerald E. Finken (RxE2 CEO) quantified this relationship, noting that pharmacists interact with patients up to 32 times a year. This frequency creates an unmatched opportunity for building and maintaining the trust needed to solve long-standing industry problems.
