Intraosseous (IO) access is an excellent way of establishing vascular access in urgent cases (trauma, burn, shock, resuscitation) when intravenous access fails or would take too long. All intravenous medications, fluids, and contrast media for imaging can be administered via IO access.

The preferred site for IO access is the proximal humerus as any fluids given via this route end up in the systemic circulation via the superior vena cava. IO/IV access in lower limbs is not recommended for trauma patients with pelvic or abdominal injuries as due to extravasation the desired response to the medication/fluids given may not be achieved.

See the short video for the essentials of IO needle placement for the proximal tibia and humerus.
