Late last year MSSC was asked to assist the Lancashire Coroner with an investigation into the cause of a fatal diving accident occurring at the Delph Dive Centre in Chorley. We provided an accident report and Chris Baldwin attended the Coroner's Inquest as an expert witness. The conclusion of the inquest is shown in the article. In this case two major issues were highlighted: firstly that divers should always Plan the Dive and Dive the Plan; secondly that the 'safety stop' is becoming a 'hard stop' during recreational and leisure diving activities and must not override higher safety priorities during an emergency. MSSC provides consultancy services including accident investigation. Please contact us if you require further information.
