Release Schedule:

Date: Starting August 10, 2026 (Monday)
Time: 9:00 AM – 4:00 PM

Graduates are requested to personally claim their Form 137 during the designated office hours. Kindly present a valid government-issued or school ID for verification upon claiming the document.

If you are unable to claim your Form 137 personally, you may authorize a representative to do so on your behalf. The representative must present the following:

A signed Authorization Letter from the student;
A photocopy of the student's valid ID; and
The representative's own valid ID.

If your Form 137 is being requested by another school, college, or university, please ensure that you present an official Letter of Request from the requesting institution before the document can be released.
