Can TCM and CCM be billed for the same patient in the same calendar month?

They can, but the record must clearly show where one service ends and the other begins.

🔵 The TCM period must have ended before month-end. At least 20 minutes of separate CCM work must then be furnished and documented, with no time shared between the services.

This rule is one example of why CCM cannot be reduced to accumulating minutes. The clinical work, documentation and billing logic all have to remain aligned.
