Marthaโs Vineyard faces significant healthcare challenges, with 8.8% of residents living at or below the poverty line. To address these inequities, the Boards of Health partnered with Health Care to deliver community-based nursing services, meeting people where they are.
In 2023, Island Health partnered with Dimagi to introduce a mobile application powered by the Omaha Taxonomy for Nursing. This tool helps public health nurses assess needs, deliver targeted care, and track outcomes, enabling evidence-based decision-making.
