Physical therapy practices lose an estimated $8.5 billion a year to patient non-compliance. Not to bad outcomes, not to litigation, not to reimbursement cuts โ€” to patients who simply stop coming.

This patient didn't say the pain disappeared. They said they got a break from thinking about it.

A patient's imaging comes back clean. Their strength tests fine. And they still can't reach the top shelf without bracing for it.

Modern pain science has a name for what's happening: the nervous system has learned to protect. Persistent pain and mobility limitation are frequently linked to changes in how the brain processes threat โ€” not to ongoing structural damage. The tissue healed. The protective pattern didn't.

This is why "just push through it" fails, and why fear-avoidance behaviour is so difficult to unlearn with instruction alone. You can't reason someone out of a response their nervous system generates before conscious thought.

What does seem to work is graded exposure: movement reintroduced in small, controlled, non-threatening increments until the nervous system updates its own prediction.
