Our Case Manager On-Demand Solution is Helping Stabilize those that are Homeless:
The Apportis platform is being used in five Ohio counties to connect to and stabilize underserved, at-risk populations including homeless youth and homeless pregnant youth. These youth may have experienced sexual violence, are LGBTQ, have mental or developmental disabilities, youth who are struggling with addiction, are transitioning out of foster care, or have had issues with the juvenile justice system. The new platform needs to plan and provide the target audience with access to stabilizing resources - including but not limited to:
1. Behavioral and physical health services
3. Education and employment opportunities
4. Community and social connections
Our solution removes the distance between vulnerable youth and LFCAA case managers and allows trust to be built on-screen with face-to-face communication. For those youth with time, transportation, and financial barriers, who have access to technology such as a cellphone, tablet, or computer, this offers innovative and immediate solutions. It also allows for social distancing during these unusual times with the COVID-19 virus. In addition to the video component, this platform will provide the youth with instant access to resources, information, and a personal self-help library via a content management system. The platform will also enable LFCAA to collect meaningful data and develop reports.
