If you're building your 2027 budget on this year's Medicare numbers, it's worth a second look.

CMS released the CY 2027 Physician Fee Schedule proposed rule in July. Payment is headed down, and beyond the headline number there are several proposed changes to how office visits, procedures, and practice expenses are valued.

What it means for your practice: this isn't a flat percentage cut that lands the same way everywhere. The impact depends on your visit mix — what you bill, how often, and in what combinations. Two practices in the same specialty can come out quite differently.

What to do before September 14:

Pull your own claims data and model the affected service lines rather than assuming an across-the-board reduction. Flag the claim types that would be touched so you can identify them later. And if something in the rule would meaningfully change your economics, submit a comment — the period closes September 14, and CMS reads what individual practices send, not just what societies file.

Every ambient AI demo works. That's what makes it a demo.

Clean room. Cooperative patient. Simple complaint. Of course the note comes out well.

If you're shopping AI-powered EHR tools this year, six questions are worth more than the demo:

1. Compared to what? Before-and-after isn't evidence. Ask if there was a control group. Usually there wasn't.

2. Which metric? Time in the note, total EHR time, and after-hours charting are three different things. A tool can improve one and worsen another.

3. What's your adoption rate? Not satisfaction — adoption. A tool nobody opens saves nobody anything.

4. What does it get wrong? Every one of these errs. A good vendor knows the error profile because they measured it. A bad one says "accuracy is high."

5. What's the edit rate? That's the honest efficiency number. A draft needing heavy rework isn't saving what the pitch claims.

6. Who signs the note? A clinician does, regardless of who drafted it. Build the review step before go-live, not after.

The evidence on ambient documentation is encouraging. It also shows real variation between tools sold on identical promises. That variation is your risk, and no demo will surface it.

Ask harder questions. Choose your vendor wisely.
