A clinic asked us this week if their new AI tool was “HIPAA.”

I asked a smaller question:

If a patient message ever hits that tool, where does the copy live after someone hits delete?

Application logs, vendor logs, backups, support tickets.

We stay on the public side of the door on purpose. Blank forms can route by appointment type and completed ones go to clinic staff through a channel they already own, we never see the note.

A Tuesday morning should still work if we are not on the call.

Then a clinic asked us to solve something real.

Not a demo. A real practice with staff, private records, clinical judgment and consequences.

We sat down with the workflow and built the concept that afternoon: clinician-directed, PHI-free by design, fictional data on every screen, and a human review gate before anything could be sent.

The receipt is on slide two.

Now a second clinician-led clinic in Newport Beach is in owner verification.

So yeah we’re making it official.

Every AI deployment plan covers what the agent will do.
Almost none cover the day it gets something wrong.

Years of clinical work taught me: after a rupture, trust is never restored by time passing or a convincing apology. It's re-earned: smaller responsibility first, observed and then gradually back.

Here's the good news about agents: they aren't people. You can pull authority in one second without hurting anyone's feelings and hand it back only against evidence.

The bad news: almost nobody designs that ladder before they need it.

Microsoft just documented an exploit where an AI agent browsing the wrong website can hand over the keys to the machine.

Read that again: the agent didn't get hacked. It got persuaded.
Prompts are requests. Boundaries are architecture.

If your agent's limits live in its instructions, you don't have limits. You have suggestions. 🌊
