The 2026 HIPAA Security Rule update reaches every medical and dental practice in the country — solo provider or 50-doctor group.

Three new requirements that most practices don't know are coming:

1. **Encryption everywhere.** PHI at rest and in transit. If your EHR vendor says "we encrypt in the cloud," that's their compliance, not yours. You're still the covered entity.

2. **Multi-factor authentication (MFA) on every system that touches patient data.** Email, EHR, scheduling, billing. No exceptions. "Strong password" is no longer enough.

3. **AI tool risk assessment.** If your front desk is using ChatGPT to draft patient emails, or your biller is pasting notes into Grammarly, that's a reportable risk under the new rule. You must document what AI tools are in use and how PHI is protected.

Most practices we talk to have #1 and #2 half-done. Almost none have #3 started.

The rule is live in 2026. OCR has already said enforcement priorities include AI tool use in covered entities.
