Windows 7:

Icons not showing up? A missing or generic icon showing up in place of the program icon? Use this little trick to get your icons back.

Open notepad
Paste the code below
Save it as file.bat on your DESKTOP
Close notepad
Double click on the .bat file you saved on your DESKTOP
Give it a couple of seconds
Icons should be back!

taskkill /F /IM explorer.exe
cd /d %userprofile%\AppData\Local
attrib –h IconCache.db
del IconCache.db

Then open explorer and navigate to C:\users\username\appdata\local\ (make sure you've select to view hidden folders) and delete IconCache.db.
