For much of this year, the expectation was simple: as supply chains stabilized, cost pressure would ease with them. That's not what we're seeing.

Construction input costs are still up 7.1% year over year. Far East ocean rates remain more than 230% above February levels. Health benefit costs are climbing past $18,500 per employee. Availability has improved — but availability was never the only driver of cost.

What's emerging instead is fragmentation: pressure that no longer moves in one direction but builds unevenly across categories, each shaped by its own regulatory, labor, or capacity dynamics. That makes blanket cost strategies less effective and category-level visibility more essential than ever.

In most large health systems, non-clinical spend has no single owner.

IT sits with one team. Marketing with another. Facilities, staffing, and professional services, each negotiated on its own. The result is fragmented leverage and contracts that rarely get benchmarked against the broader market.

WellSpan Health, a 250-location system across Pennsylvania and Maryland, decided to change that. Working with LogicSource, it treated more than $300 million in non-clinical spend as one opportunity instead of a set of separate administrative tasks.

Most health systems apply real supply chain discipline to clinical spend. Non-clinical categories tend to get far less attention, even though they can reach roughly 20% of revenue.

Software licensing sits with IT. Agency contracts sit with marketing. Staffing agreements sit with HR. Each is negotiated in isolation and measured against a healthcare-only frame of reference, which quietly caps how much value the benchmark can reveal.

WellSpan Health took a more deliberate approach. Working with LogicSource, they began evaluating non-clinical categories against retail, finance, and manufacturing standards rather than hospital peer data alone, then built an embedded partnership model to act on what that data showed. The result was more than $35M in annual savings within three years, with no impact on clinical care.

Say you're planning a big trip. Do you use AI to lock in the flights, hotels, and reservations before you leave — or just pull it out at each stop to sort things as you go?

Obvious. You want the plan set before departure.

Most companies are doing the opposite with procurement AI. They've aimed it downstream — invoice matching, PO processing, payment workflows. Useful work, but it only speeds up decisions already made. By the time spend reaches that layer, pricing, scope, and terms are locked.

The leverage is upstream, at source-to-contract, where supplier economics are shaped, and commercial risk is negotiated. Or as Jo puts it: AI downstream optimizes the mechanics; AI upstream improves the economics.

A vendor contract sits in negotiation for six months. A capital request cycles through committee after committee. Everyone knows the decision needs to be made — and still it waits.

LogicSource advisor Mark Van Sumeren calls this the absence of a forcing function: governance that was never built to bring a decision to a close. His argument is that the delay isn't a failure of leadership — it's a failure of structure, and it quietly consumes the capacity health systems can least afford to lose.
