Would you be confused if you had to complete a form like this? Believe it or not, this is just part of an actual form from a local city tax website. As a frequent user, I’m thrown off every time (and still don’t know my account number or municipality). While tax forms aren’t necessarily an exciting user experience design problem, they sure are an important one.
