Connecting with our community for better health! 🤝

We recently completed another successful session of our Special Diabetes Program for Indians (SDPI) education series, this time presenting to the wonderful seniors at the Shinnecock Indian Nation.

Our presentation focused on Physical Fitness for Seniors. We discussed how incorporating regular physical activity—like balance exercises, stretching, and achieving 150 minutes of movement per week—is a crucial step in managing and preventing diabetes.

The dialogue and interaction with the seniors were truly inspiring, and we are proud to support community-led health initiatives. Our goal is to continue providing relevant, accessible, and respectful health education to empower everyone to live healthier lives.

Yesterday, our team at Vital Care Services had the distinct privilege of hosting Dr. Brian Goldstein, Dean of the Pace University College of Health Professions, at one of our Westchester County TIPS sites at the India Center of Westchester for a site visit.

Having a leader of Dr. Goldstein's caliber conduct a site visit speaks volumes about his commitment to community health and innovative new models of care like TIPS. It was inspiring to see him actively engaging with our setup and speaking with our participants & staff (Social Service Assistant, Marilyn pictured here)

We have deeply valued our partnership with Pace University over the past 10+ years. As one of our co-founders are Pace alumnus, seeing this collaboration continue to thrive and impact the community is incredibly meaningful. Investing in the health of our community! 💙

We had a wonderful day spent with the Indian Health Service Special Diabetes Program for Indians (SDPI), talking about one of our favorite topics: Food as Medicine.

As we get older, our bodies change, but our need for great nutrition stays the same. We discussed how simple changes in what we eat can help:
✅ Keep our energy steady throughout the day.
✅ Protect our heart and joints.
✅ Keep our muscles strong and active.

But we didn't just talk! Our team was on-site providing clinical screenings to make sure everyone has the data they need to stay healthy and independent. 🩺✨

We are so grateful to Shinnecock for inviting us into their space. It’s an honor to support tribal wellness and see neighbors taking charge of their health!

Healthy eating doesn’t have to mean complicated meal prep or strict rules! 🥦

One of the easiest ways to improve your daily nutrition and support your long-term health is the "Plate Method." Next time you sit down for a meal, try to visualize your plate like this:

✨ 1/2 of your plate: Non-starchy vegetables (like leafy greens, broccoli, or peppers)
✨ 1/4 of your plate: Lean protein (chicken, fish, tofu, or beans)
✨ 1/4 of your plate: Complex carbohydrates (brown rice, sweet potatoes, or whole grains)

We are proud to officially launch our newest initiative: the Shinnecock Indian Nation SDPI Program! 🎉

In partnership with the Indian Health Service’s Special Diabetes Program for Indians (SDPI), we recently wrapped up our very first monthly in-person diabetes education workshop at Shinnecock. We had an engaged group of older adults join us for a deep dive into the critical relationship between hypertension and diabetes.

Our objective is to track real progress and show our impact; following the workshop, we provided free health screenings for our attendees. Moving forward, tracking blood pressure will be our key measure as we work together to help improve participants' numbers month over month.

We will be hosting these in-person workshops monthly, so stay tuned for our next date in April.

Last week Vital Care Services had the amazing opportunity to send representatives to go to the Special Diabetes Program for Indians (SDPI) 2024 Grantee Conference in New Mexico. We were able to share our plan and future goals to the committee, and also explore what Albuquerque has to offer. We’re so excited to see how Vital Care Services continues to grow, and expand our services!

As you know, VCS is committed to serving all types of communities, and are proud to work with such a diverse community. We are also dedicated to the Native American population in the United States. In the United States, Native Americans and American Indians have the highest rates of diagnosed diabetes, at a rate double that of non-Hispanic whites. VCS is committed to serve these communities to ensure everyone receives the knowledge and help they need to live their healthiest lifestyle.

Without a doubt the week went exceptionally well, and we loved experiencing the beauties of Albuquerque!

Did you know that Vital Care Services also provides at home service?

Participants can enroll in two at home services:
1) Remote Physiological Monitoring with their hospital physician group / homecare
2) TIPS At Home service

Participants in these services will receive their own Telehealth kit where they take their vitals from the comfort of their own home. These vitals data are then sent to their primary physician/NP (RPM) or a triage nurse who will determine what further actions to take (TIPS).

If there are any alerts noticed in vitals measurements, your nurse/physician can contact participants directly. If it's noted that the participant's vitals are normal over a long period of time, they can even be discharged from the program. Free return labels are provided with each kit.

Enrolling with VCS comes at NO COST for the patient, and is COMPLETELY VOLUNTARY. Tech support is available for participants who further assistance and can be done via phone call or video call (through VCS patient portal). Additionally, all patient data is confidential, and will only be seen by clinicians.

What's included in the kits?
1) 1) Tablet and charger - All devices are Bluetooth paired and data will automatically be uploaded to the tablet.
2) Blood Pressure Cuff - Wireless cuff so participants can easily slide in their arms to take vital measurement. Universal adult cuff size.
3) Blood Oxygen Monitor - Pulse + Oxygen level readings
4) Scale - Body weight
5) All devices are sent in a gray suitcase for ease of storage
