That incident was not a model failure. The model performed exactly as it was designed to.

The failure was in the system around it. No destructive-action controls. No explicit boundaries defining what the agent was and was not permitted to do. No recoverable ex*****on path for when something went wrong.

As AI moves from generating outputs to taking actions inside enterprise systems, this is the risk that most organizations are not adequately prepared for. Traditional software is deterministic. Behavior is encoded in code. When something breaks, you trace the logic.
