Ask any private duty nurse about documentation and you will hear the same thing: it is not the charting that is hard, it is charting in a house with one bar of signal.
When the system needs a connection, nurses fall back to paper notes and re-enter everything at the end of a 12-hour shift. Detail gets lost. Times become approximations.
